License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: All nurse assessors must meet the following requirements: - Two (2) years of experience in a HOME setting within the last ten (10) years. - Experience conducting comprehensive assessments of patients. Description: Required licensure includes current RN licensure in Client. Candidates must have a minimum of two years of experience working in a home setting within the last decade. Recent experience conducting comprehensive patient assessments is essential. Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days. Experience caring for a geriatric population and managing chronic diseases in a home setting is also preferred. Combination home care experience as a paraprofessional or licensed practical nurse caring for a minimum of two patients per week is ideal. This position requires the ability to review health status questions and make evaluations based on UAS criteria. Candidates must be comfortable working with technology in the workplace. An understanding of nursing theories, principles, and practices is necessary. Strong organizational, interpersonal, written, and verbal communication skills are required. Candidates must demonstrate sensitivity to the concerns of diverse consumers. The ability to perform effectively in a fast-paced, deadline-oriented environment is essential.
