Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 8x5 Days Hours per Shift: 8 Experience: Two (2) years of experience in a HOME setting within the last ten (10) years. License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: All nurse assessors must meet the following requirements: - Two (2) years of experience in a HOME setting within the last ten (10) years. - Experience conducting comprehensive assessments of patients. Description: Required licensure includes current RN licensure in Client. Candidates must have a minimum of two years of experience working in a home setting within the last decade. Recent experience conducting comprehensive patient assessments is essential. Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days. Experience caring for a geriatric population and managing chronic diseases in a home setting is also preferred. Combination home care experience as a paraprofessional or licensed practical nurse caring for a minimum of two patients per week is ideal. This position requires the ability to review health status questions and make evaluations based on UAS criteria. Candidates must be comfortable working with technology in the workplace. An understanding of nursing theories, principles, and practices is necessary. Strong organizational, interpersonal, written, and verbal communication skills are required. Candidates must demonstrate sensitivity to the concerns of diverse consumers. The ability to perform effectively in a fast-paced, deadline-oriented environment is essential. Candidates should be capable of executing complex tasks simultaneously and working independently. Job responsibilities include performing initial in-home evaluations of patients that encompass health status assessments, care needs, and preferences. Nurses will review consumer medical documentation relevant to each case. Data entry into electronic systems and documenting any concerns during evaluations is part of the role. Communication with consumers regarding health care options and service vendor choices is crucial. The position involves enhancing collaboration with interdisciplinary care team members to improve care coordination. Collecting quality review data and maintaining knowledge of community resources is required. Identifying opportunities for health promotion and illness prevention is encouraged. Nurses will meet performance standards established for this position and perform other duties as assigned.
