Job Title: Travel Home Health Nurse

Job Summary:The Travel Home Health Nurse prescribes, coordinates, and delivers care through the nursing process, maintaining the standards of professional nursing practice. This role involves supervising CNAs and coordinating care with other healthcare workers as assigned to ensure comprehensive, patient-centered care.

Key Responsibilities:

• Conduct comprehensive initial assessments for all clients and continuously assess physical, psychosocial, environmental, and community resources during subsequent visits, applying principles of growth and development across the lifespan.
• Develop, initiate, re-evaluate, and modify Plans of Care as ordered or needed.
• Establish discharge plans collaboratively with patients, physicians, interdisciplinary team members, and caregivers.
• Complete OASIS data in compliance with regulatory requirements.
• Provide patient care adhering to current Federal, State, and Agency regulations.
• Assess educational needs of patients, families, and caregivers and implement appropriate teaching strategies.
• Complete accurate and timely documentation following regulatory guidelines.
• Promote a clean and safe environment of care, utilizing SAFE error prevention habits.
• Uphold the highest standards of privacy and confidentiality regarding patients, coworkers, and hospital operations, in line with Standards of Conduct.

Submission Requirements:

• Home Health experience – REQUIRED
• Candidates with strong Med-Surg experience and some Home Health experience will be considered
• OASIS and start-of-care experience – strongly preferred
