Job Title: Ambulatory Care Nurse Profession: Nursing Specialty: Ambulatory Care Duration: 13 Weeks Shift: Day Shift Hours per Shift: 8 hours Experience: Minimum 2 years required License: Registered Nurse License Certifications: BLS, ACLS Must-Have: Minimum of 2 years of experience BLS and ACLS certifications Description: In this role, the successful candidate will support the coordination, integration, communication, implementation, and evaluation of nursing practice and clinical policies and procedures. The Ambulatory Care Nurse will participate in site and organizational quality and performance improvement activities. This includes collecting patient healthcare data systematically using appropriate assessment techniques and instruments. The nurse will collaborate with licensed providers to develop patient-centered plans of care that prescribe interventions to attain expected outcomes. The Ambulatory Care Nurse provides comprehensive care in support of all patients. This includes the administration of treatments and medication, acting as a sentinel for untoward events or symptoms, disease prevention guidance, rehabilitation care, public health care, and supportive care for symptom relief, including health teaching and counseling. Physical examinations and diagnostic tests will be performed as needed. The nurse will collect patient health histories and record information in the electronic health record. Information, counsel, and education regarding test results and treatment plans will be provided to patients and families as directed. Medications will be administered, including vaccinations, IV therapy, and oxygen administration. Comprehensive wound care will be provided, along with phone and in-person patient triage for clinical concerns. Patient responses to various treatments, procedures, and anesthesia will be assessed and monitored. The nurse will assess and interpret patient information, making treatment decisions as appropriate. Collaboration with clinical supervisors and licensed providers will be undertaken to determine best treatment plans for patients. The nurse will direct, supervise, and delegate tasks to licensed practical nurses, medical assistants, and technicians as needed. Other duties will be performed as necessary. Clinical compliance with patient safety initiatives and reporting will be ensured. Key quality initiatives will be supported to impact clinical outcome measures. The nurse will demonstrate knowledge of service standards and incorporate them into the performance of responsibilities. Participation in designated activities related to clinical standards and relevant regulations will be expected. The nurse will participate in the coordination of on-site activities to provide patient care in a healing, humane, and caring environment. Collaboration with members of the interdisciplinary team and promoting collegial relationships will be essential. The nurse will serve as a support and resource for nursing, other clinical staff, and providers. Participation in clinical inquiry and activities to foster evidence-based practice will be encouraged. Emergency care will be provided in accordance with organizational policy. Respectful membership within the healthcare team will be modeled, and specific nursing interventions will be delegated and overseen as appropriate. Self-assessment and participation in activities to promote professional growth and development will be essential. Maintaining current nursing knowledge and competence to fulfill responsibilities will be expected. The nurse will procure and apply new knowledge related to medical treatment and technology. Resources will be utilized to meet individual learning needs and promote professional development in clinical practice. Awareness of current patient-centered, evidence-based practice will be maintained. Mentoring of peers and colleagues will be encouraged. The obligation to identify and report professional misconduct will be fulfilled. Implementation and provision of care will be executed as prescribed by licensed providers. Assessment of patients will be conducted to identify unmet needs and plan comprehensive care. Integration of care with other healthcare providers will be necessary. Direct care services will be provided, including administration of treatments and medications, disease prevention guidance, rehabilitation care, public health care, and supportive care. Patient clinical historical and assessment data will be collected and documented. Onsite testing, such as point-of-care lab testing and specimen collection, will be performed as needed. Medical and nursing treatments will be executed in accordance with the plan of care and provider orders. Procedures such as wound care, dressing changes, and monitoring will be performed. Medication administration will occur according to provider orders and organizational policies. Communication and message management will be handled effectively. In-basket management and timely closure of encounters will be ensured. Critical lab values will be documented and reported as indicated. Results of clinical assessments will be relayed to patients, with follow-up information provided as directed by the provider. Care and case management for a defined population will be facilitated as needed. Health promotion and education will be prioritized. The nurse will identify deviations from health and well-being at the patient, family, and community levels. Patient needs and barriers will be assessed, and individualized plans of care will be developed. Patient and family education will facilitate healthy adjustments to new health patterns.
