All Medical Personnel - Nursing is seeking a travel nurse RN Clinic for a travel nursing job in Minneapolis, Minnesota.

Primary Responsibilities:

Clinical Care & Coordination

• Manage patient phone triage and walk-in concerns
• Communicate abnormal labs and provider-directed orders
• Administer vaccines as needed and coordinate vaccine-related programming
• Manage prescription refills and durable medical equipment orders
• Triage and process home health and external documentation
• Respond to medical emergencies within the clinic
• Maintain crash cart, medication disposal, and sample meds

Care Management & Population Health

• Conduct nurse-led visits under standing orders (intake, physicals, follow-ups)
• Track patient engagement, manage appointments, referrals, and hospital transitions
• Assist patients in accessing community resources (WIC, dental, nutrition, social services)
• Conduct population outreach to promote care continuity
• Maintain accurate and timely documentation in the EMR

Team Collaboration

• Attend planning meetings and contribute to nursing performance improvement projects
• Participate in cross-functional work groups as needed

Required:

• Active, unrestricted RN license in Minnesota
• BLS/CPR certification
• Minimum 2 years of direct patient care experience in a clinic setting
• Proficient computer skills: EMR, Outlook, Word, Excel, PowerPoint

Preferred:

• Prior experience with Native American or underserved communities
• Familiarity with population health, chronic disease management, and social services navigation
