The RN will work in patient management focused on effective care coordination. The role requires collaborating with healthcare professionals for discharge planning. Candidates must have a strong understanding of community agencies. A minimum of one year of relevant nursing experience is required. An active RN license in New York and Basic Life Support certification are mandatory. This position has a standard schedule of Monday through Friday, no weekends or holidays. Candidates residing within 50 miles of the facility are not eligible.
