Job Title: Case Manager Profession: Case Management Specialty: Behavioral Health Duration: 9 months Shift: Hybrid Hours per Shift: 35 hours per week Experience: One year experience License: Required Certifications: Required Must-Have: BA required Description: Accountable to prepare, organize, and assist Case Managers. Coordinate and maintain all office management functions. Independently manage patient discharge functions such as transportation and nursing home returns. Intervene with insurance companies on utilization review activity. Promote access to and oversee the coordination and delivery of comprehensive, quality healthcare services for members. Perform telephonic care management activities including assessment, plan of care development, care coordination, outpatient follow-up, and ancillary service review to ensure optimum health outcomes. Develop and implement an individualized, member-centered Plan of Care through direct telephonic and face-to-face member engagement. Include, where appropriate, collateral contacts such as family members (with appropriate consent of the member), primary care provider, and other community and case managers to identify and address the needs and barriers of members to promote health and wellness. Provide health education using coaching and motivational interviewing techniques to improve health outcomes. Engage in care and viral load suppression, behavioral health and substance use services, and effective management of other co-morbid illnesses. Participate in special outreach and quality improvement projects as assigned. Document all care management activities for each member and ensure compliance with professional standards and regulatory guidelines using the correct templates. Address member primary care provider assignments during outreach and reassign members as appropriate. Carefully follow designated departmental compliance, care management, and quality improvement workflows as assigned. Escalate cases and issues to supervisors as appropriate. Answer calls on the inbound call center telephone line in a professional and timely manner. Attend and prepare for case conferences, including interdisciplinary case conferences and conferences on special topics. Orient assigned new members to the scope of services provided as needed. Ensure coordination of membership enrolled in all lines of business is compliant with Federal, State, and City regulations.
