Case Manager RN needed to coordinate utilization management, discharge planning, resource management, and care facilitation across the acute care continuum. The RN will manage high-risk patients, focusing on reducing unnecessary admissions/readmissions, ensuring appropriate post-acute care referrals, and minimizing payment denials. Role includes inpatient and ED coverage.

Key Responsibilities:

• Perform utilization review using InterQual and Milliman guidelines.
• Coordinate discharge planning and post-acute care referrals.
• Facilitate care transitions for complex patient populations.
• Support emergency department patients requiring discharge planning.
• Collaborate with providers, interdisciplinary teams, and regulatory bodies (CMS, DOH, Joint Commission).
• Float between inpatient and ED case management as needed.
• Work with adult and geriatric populations, average daily census 100–120.Ax
