Certification Requirements:CA RN LICENSEBLS

Experience: 5+ Years of RN Case Management experience.

Role Purpose:Coordinate patient care and transitions throughout the hospital stay to ensure safe, timely discharge and prevent unnecessary admissions or readmissions.

• Assess patients' medical, psychosocial, and financial needs
• Develop and lead individualized care and discharge plans
• Monitor level of care (LOC), length of stay (LOS), and avoid delays
• Collaborate with physicians, nurses, social workers, and payers
• Arrange follow-up care, community services, and resources
• Address barriers to discharge and reduce readmission risk
• Document all plans and communications in the EHR
• Stay current with CMS and Joint Commission regulations
• Participate in rounds, team meetings, and process improvement
