👤 Role Overview
- Focus: Inpatient Discharge Planning (NOT Utilization Review)
- Primary Units: Medical-Surgical (MS) or Telemetry (Tele)
- Collaborates with: Social Worker (SW), Physicians, and Interdisciplinary Team
- Expected Duties:
- Rounding with physicians/team
- Coordinating referrals (e.g., SNF, housing, hemodialysis, wound care/KCI)
- Discharge planning to home, community, or facility
- Managing complex social needs (e.g., homelessness support)
- Must be:
- Independent
- Critical thinker
- Organized with strong time management
- Team-oriented with a positive attitude

- Typical floor census: 33–40 patients
- Staffing:
- Standard: 1 CM + 1 SW → Caseload: 17–25 patients
- Pilot Floors: 1 CM + 2 SWs → Caseload: 12–17 patients
- Minimum expectation: Ability to handle at least 20 patients
- Common coverage: Internal Medicine and Family Medicine patients
