📋 Unit Information:

• Department: Care Management (Utilization Review)
• Setting: Acute Care
• Charting System: Epic
• Special Note: Discharge experience required

✅ Requirements:

• NY RN license (BSN not required)
• BLS (AHA) certification
• PRI Certification (within 3 months of hire)
• Screen certification preferred
• Care Management certification preferred (CMC, CCM, etc.)
• 2+ years' RN experience (3+ years preferred in ED, Critical Care, or Med-Surg)
• Previous Case Management experience preferred
• Strong knowledge of healthcare financial, regulatory, and payer issues
• Familiarity with Milliman/InterQual criteria
• Excellent computer skills

📝 Role Overview:As an RN Care Manager, you will perform comprehensive admission assessments, identify high-risk patients, coordinate multidisciplinary care, and plan for safe, effective discharges. You will conduct Utilization Review activities, collaborate with healthcare teams, and ensure department goals for LOS reduction, readmission prevention, and patient satisfaction are met.
