Aya 2982852This is a hospital-based acute care case management role requiring 2+ years of recent experience in discharge planning and care coordination. The RN will handle a caseload of approximately 20 patients, conduct utilization reviews (InterQual proficiency required), and independently coordinate post-acute needs such as DME, SNF placement, and ambulance transportation.

Responsibilities include:

• Patient assessments and care planning
• Collaboration with social services, transportation, and clinical teams
• Organizing family meetings and participating in treatment planning
• Working with HMOs/IPAs and understanding CMS, DRG, and regulatory compliance
• Addressing patient needs related to substance abuse, mental health, homelessness, and protective services

EPIC and Microsoft Office proficiency are required.Weekend coverage is required every other weekend.Travelers must be experienced, but first-time travelers may be screened.Ax
