eTeam Inc - Secondary is seeking a travel nurse RN Case Management for a travel nursing job in Bronx, New York.

Job Title: RN Case Manager – Bronx, NYShift: Monday to Friday, 8:00 AM – 4:30 PM (7.5-hour shifts with 1-hour lunch)

Required Certifications:

• BLS (AHA)
• PRI & SCREEN certifications
• RN License of NY State

Experience Requirements:

• Minimum 1 year of inpatient case management experience
• Prefer prior critical care background
• Knowledge of discharge planning and community resources
• Prefer familiarity with CarePort, MCG, and/or InterQual
• Prefer some utilization review experience

Role Overview:

• Focus on discharge planning for patients on Med-Surg or Tele floors
• Work collaboratively with Social Workers, physicians, and interdisciplinary teams
• Handle a caseload of 17–25 patients (pilot floors may have fewer)
• Coordinate post-discharge needs such as home care, SNF placement, shelter referrals, wound vacs, and hemodialysis arrangements
• Expected to function independently while maintaining strong communication and time management skills
