Unit - Case Management: Case Management (Utilization Review)

Shift - 8H Days 5 - 8.5hr Shifts with 1hr lunch

Guaranteed Hours - 37.5 hours per week

Description - Unit Notes:Department: Case Management *Required Certifications: BLS*

- Other Certifications/Specialty certifications requirements?: PRI & SCREEN certifications (can be taken online through the School of Albany- typically take a couple hours for each certification;

- Role: Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker

Case load: Floors typically have 33-40 patients; Most floors are split with 1 CM & 1 SW; Caseload: 17-25 patients. Pilot Floors will have 1 CM & 2 SWs; Caseload: 12-17 patients. Need to be able to handle at least 20 patients. Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med.
