Requirements:

• Current NYS RN license

• Minimum 2 years of nursing experience (ED, critical care, or med/surg strongly preferred)

• BLS (AHA) required

• PRI certification (must be obtained within 3 months of employment)

• Screen certification strongly preferred

• Previous case management experience preferred

• Care Management certification (CMC, CCM, etc.) preferred

• Experience with Milliman/InterQual criteria

• Strong knowledge of healthcare financial, regulatory, and payer issues

• Knowledge of state, local, and federal programs strongly preferred

• Proficiency in Epic and strong computer skills

Unit/Role Notes:The RN Care Manager is responsible for comprehensive patient assessments, utilization review, and discharge planning. This includes screening patients using established tools for high-risk indicators, coordinating care across disciplines, and ensuring safe and appropriate discharge planning.

Key Responsibilities:

• Complete initial and ongoing patient assessments

• Perform multidisciplinary care coordination at both intra- and inter-hospital levels

• Manage all transitional planning activities including patient/caregiver education, aftercare arrangements, and payer communication

• Collaborate with health team members and ensure effective communication of patient needs

• Conduct utilization review activities per department policy (payer reviews, admission clinical reviews, variance management)

• Support department goals of LOS reduction, readmission prevention, denial reduction, and patient satisfaction improvement

• Participate in performance improvement initiatives

Preferred Background:

• 3+ years of ED, critical care, or med/surg experience

• Prior case management experience in an acute care hospital

• Strong communication and interpersonal skills
