Registered Nurse - Case Manager - Weekend Requirements: Every other weekend COVID-19 Vaccine: Not Required Skills: Acute Hospital, Care coordination, CMS: Centers for Medicare and Medicaid Services, Discharge Planning, DRG (Diagnosis Related Groups)*, Plan of Care, Utilize InterQual Criteria, Extra Checklist w/ required skills required

Unit Highlights -Patient Ratios Days: 20 Orientation Hours: 40 Facility & Patient Care Details Patient Age Groups: Adolescents, Adults, GeriatricsPatient Diagnoses: Utilization Review knowledge is a Hardstop Special Procedures/Unit Details: Admission Criteria (InterQual) (ED only) Experience with Assessing patients, Care Coordination, and Discharge Planning Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc. Ability to organize and participate in family meetings. Participate in the development of the treatment plan. Experience with HMOs, IPAs, etc. Competent with arranging DME, SNF and ambulance transport. Must be able to perform this task independently. Special Equipment : Computerized documentation, required.
