HIGHLIGHTS:

• Minimum 2 years of Home Health experience strongly preferred
• Travel experience required
• Required Certs: WI License, AH ACLS, Valid Driver's License, OASIS cert is a plus
• EPIC and OASIS experience strongly preferred
• Weekends: Every other
• On Call: 1-2x during the week and every other weekend
• Float Requirements: potential to float to other territories to assist when needed during low census
• Contract Length: 13 weeks
• Guaranteed Hours Per Week: 40
• Shift: 8hr Days
• Shift Time: 8a-430p - potential to flex start and end times if needed
• Required Education: Associate Degree or Certificate
• Holiday Rotation Requirements: Rotation
