Key Requirements:

• Minimum of 2 years of hands-on case management experience (utilization review experience does not meet this requirement)
• Active Michigan RN or LMSW license

Preferred Qualifications:

• Prior experience using Care Manager and Epic EMR systems
• Background in discharge planning
