Travel RN – Case ManagementLocation: Jackson, MSStart Date: 9/1/2025Duration: 13 weeksSchedule: Monday–Friday, 8:00 AM – 4:30 PM (5x8s, non-negotiable)Guaranteed Hours: 40 per week

Care Setting: Acute CareCharting System: Epic

• Active RN license (will wait for Mississippi license if needed)

• Minimum 2 years' acute care case management experience with discharge planning

• At least 6 months at a Level 1 Trauma Center or large academic medical center

• Epic charting experience required

• BLS required (AHA)

• CCM preferred, but not required

• BSN preferred; ADN considered

• Focus: Case Management – Discharge Planning (not UR)

• Patient Load: 15–20 patients/consults per day

• Coverage: All inpatient units

• Perform comprehensive discharge planning for a wide variety of patients

• Work with medical discharges and occasional social discharges

• Collaborate with interdisciplinary team including social work, RT, and pharmacy

• Level 1 trauma hospital serving complex patients with limited placement options

• Weekend Requirements: None

• Holiday Requirements: Rotating, as needed

• Case Management Certifications: Preferred but not required
