• Coordinate patient care plans across the continuum, from admission to discharge, ensuring cost-effective and quality outcomes.
• Perform comprehensive assessments to determine patients' needs, eligibility for services, and appropriate care levels.
• Collaborate with physicians, nurses, social workers, and other healthcare providers to develop individualized treatment and discharge plans.
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed based on patient progress.

Requirements:

• Experience: 2+ Years
• Certifications: BLS, RN License,
• PRI
