Job Description:Join the Assertive Community Treatment (ACT) team at Jacobi Medical Center, providing compassionate psychiatric care in a community-based setting.
Key Responsibilities:
- Conduct psychiatric and medical assessments for patients admitted to ACT
- Maintain a therapeutic milieu conducive to mental health and healing
- Participate in group sessions and multidisciplinary team conferences
- Serve diverse age groups: Adolescents (12–21 yrs), Adults (22–65 yrs), and Geriatrics (65+ yrs)
- Attend daily team meetings at 3 PM to plan next-day visits
- Visit up to 6 patients per day (varies based on treatment needs, including long-acting injectables)
- Travel within the Bronx only – transportation provided by facility
- Minimum 2 years of psychiatric nursing experience
- BLS (Basic Life Support) certification
- CPI (Crisis Prevention Intervention) certification
Facility vehicles provided – no personal transportation required.
