Job Summary:
BronxCare Hospital Center is seeking experienced and self-motivated Registered Nurses (RNs) for its Inpatient Case Management Department. The RN will work closely with patients, families, interdisciplinary teams, and social workers to develop and implement effective discharge plans. The ideal candidate will be independent, organized, and comfortable managing a fast-paced caseload on medical-surgical or telemetry units.
Key Responsibilities:
- Collaborate with physicians, social workers, and care teams to assess patients and plan for safe discharges
- Round daily with interdisciplinary teams to identify discharge needs
- Make timely referrals for services such as SNFs, home care, housing assistance, and wound care
- Manage caseloads of 17–25 patients (or 12–17 patients on pilot floors with additional support)
- Educate patients and families on discharge options and follow-up care
- Coordinate with social workers and other staff to ensure effective transitions of care
- Utilize Allscripts EMR system to document assessments, plans, and outcomes
- Ensure compliance with internal discharge planning procedures
Required Experience & Skills:
- Minimum of 1 year of inpatient case management experience
- Strong critical thinking, time management, and communication skills
- Ability to function independently and collaborate effectively in a team
- Prior critical care experience preferred
- Familiarity with discharge planning workflows for inpatient populations
- Experience with CarePort, MCG, and/or InterQual preferred
Licensure & Certifications:
- Active RN license in New York State (Required)
- Bachelor of Science in Nursing (BSN) (Required)
- Basic Life Support (BLS) Certification (Required)
- PRI (Patient Review Instrument) Certification (Required – can be completed online)
- SCREEN Training Certification (Required – can be completed online)
Additional Details:
- Dress Code: Business casual or scrubs (any color except royal blue or maroon preferred); lab coat provided
- Orientation:
- 2 days hospital-wide orientation (includes EMR training)
- 1–2 weeks with a preceptor, depending on individual progression
- Weekend/Holiday Work: Occasionally required with manager approval
- Supervision: Reports to Case Management Manager, Reiny
Common Patient Discharge Scenarios:
- Discharges to home, community, or skilled nursing facilities
- Coordination of shelter/homeless housing packets
- Hemodialysis and KCI wound vac referrals
- Utilization of a resource binder for support and guidance
