Requirements:

• Active LPN license LPN license in Michigan
• Current BLS certification
• Minimum 1 year of LTC or SNF experience preferred
• Ability to work overnight shifts and block schedules
• Strong clinical and communication skills

Responsibilities:

• Administer medications and treatments as ordered
• Conduct routine health checks and monitor resident conditions
• Document care and updates accurately in the EMR
• Assist with ADLs and collaborate with the care team
• Ensure compliance with HIPAA, infection control, and safety standards
• Respond to urgent health situations promptly
• Perform other duties as assigned by the supervisor
