• Setting: Home Health (adult patients)
• Census: ~250 across all providers
• Visits per Day: Variable by caseload
• Charting System: EPIC (experience required)
• Support: Interdisciplinary team, may float if needed

• Experience: Minimum 2 years recent Home Health (must be within last year)
• Travel Exp: Preferred but not required
• Licensure: VA LPN license (or Compact, must be active at submittal)
• Certifications: BLS
• References: 1 supervisory reference required for submittal (2 for compliance)

• Blood glucose monitoring/testing
• Communication of patient status changes to RN/MD
• End-of-life care
• Intake & output monitoring
• Isolation precautions
• Pain assessment & management
• Patient history/med reconciliation
• Pharmacy calls/refills
• Vital sign monitoring

• Locals accepted at same rate
• COVID vaccine not required | Flu vax status unknown
• Interview process: All managers conduct phone interviews – prep candidates in advance
• Restrictions:
  • Cannot work Riverside Travel & Riverside Per Diem simultaneously
  • Prior perm employees must be gone ≥6 months before being rehired
• RTO: Max 7 days, no calendar week off. Must be declared upfront.
