- Certification Details
- Registered Nurse licensure
- Social Work licensure
- Certification in Case Management or Rehabilitation (e.g., CCM, ARN, ACM, RN-BC)
- Job Details
- Must have case management experience in an acute care setting
- Lead Case Manager role in Case Management specialty
- Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred
- Previous leadership or supervisory experience strongly preferred
- Basic computer knowledge and appropriate communication skills required
- Must communicate effectively in English (verbal and written)
- Personality fit: cooperative and team player
- Common diagnoses/treatments related to rehabilitation and case management
- Job Requirements
- Current Registered Nurse or Social Work licensure or Masters or Bachelors in Social Work or healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist
- Certification in Case Management or Rehabilitation preferred, e.g., Commission for Case Manager Certification (CCM), Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM), or Board Certification in Case Management by the ANCC (e.g., RN-BC)
- Masters or Bachelors in Social Work required
- Schedule Information

- Weekly guaranteed hours: 40
