13 weeks // 5x8 daysShifts: 800-1630 (8A-430P) 5 x 8 DaysOn call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as neededFloating Requirement: no floating requirementLocal Rate withing 60 miles of facilityRequirements:Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already*DC RN Licensure - ** WILL ACCEPT PENDING DC LINCENSURE WITH PROOF OF APPLICATION* OR candidate can hold LMSW/LGSW/LCSWBLS2 years of experience: discharge planning to include: admission criteria review, benefits eligibility, care coordination, conducting needs assessments and order DME; experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, Acute hospital experience; prior charge experience preferred- completion of at least 1 travel assignment*Regulatory Skills required: CMS, HIPPA, JCC; workers compensation preferred****This facility requires that all employees have the Flu vaccine***
