**Job Title: Home Health Physical Therapist****Location:** El Centro, CA**Job Description:**We are seeking a skilled Home Health Physical Therapist to provide patient care in their homes. This role requires expertise in home health and proficiency in HCHB/OASIS documentation.**Key Responsibilities:**• Deliver quality physical therapy to home health patients.• Complete same•day documentation using HCHB.• Manage 20•25 cases weekly, achieving 5•7 productivity points daily.• Attend weekly in•person office meetings.• Offer flexible care hours to meet patient needs.**Requirements:**• Valid PT License and BLS Certification.• 2+ years of home health experience.• Proficient in HCHB/OASIS, including Oasis E.• Maintain a minimum average of 26•28 productivity points.• Reside within 30 minutes of the territory or arrange nearby housing.• Reliable transportation for patient visits; mileage reimbursed.**Additional Information:**• 13•week contract, 40 hours per week.• No on•call or overtime.• Pre•approved time off required.• Includes 2•3 days of orientation and team introduction.Join us to make a meaningful impact through compassionate physical therapy care.
