This role involves evaluating patients, creating care plans, and delivering direct rehabilitation services in patients' homes. Ideal for an experienced OT passionate about providing personalized, community-based care.Key Responsibilities:Conduct evaluations and establish care plans for patients in a home settingProvide skilled therapy services per physician orders and care plansDocument evaluations, treatment progress, and any adverse eventsCoordinate with multidisciplinary teams and attend care conferencesPerform supervisory visits for OTAs and CNAs as requiredParticipate in Quality Assurance and Performance Improvement initiativesMaintain accurate records for visits, mileage, and schedulesPlan efficient patient care routes and maintain daily communication with scheduling teamMaintain licensure and ethical practice in accordance with state board standardsRequirements:BS or higher in Occupational Therapy from an accredited programActive Montana OT License – RequiredBLS Certification – RequiredMinimum 2 years of OT experience – RequiredValid driver's license and personal auto insuranceHome health experience and OASIS familiarity preferred
