• Evaluate and assess patients to develop effective, individualized treatment plans
• Provide therapeutic exercises and interventions to improve patient mobility and strength
• Monitor and document patient progress, modifying treatment plans as needed
• Collaborate with physicians, nurses, and other healthcare professionals
• Educate patients and caregivers on home-based rehabilitation techniques
• Ensure compliance with facility and state regulations in documentation and patient care
• Participate in interdisciplinary team meetings to discuss patient care plans

Qualifications & Requirements

• Active Tennessee Physical Therapist (PT) license
• Current BLS certification (AHA or equivalent)
• 1 year of recent clinical experience preferred
• Excellent communication and critical thinking skills
• Ability to work both independently and collaboratively
