Job Overview

ACI Health is seeking a compassionate and experienced Home Health Physical Therapist to join our team in the scenic Seaside, Oregon region. This role is ideal for dedicated professionals who enjoy providing one-on-one patient care in the comfort of patients' homes. You will work with a variety of clinical conditions, ranging from post-surgical recovery to neurologic and orthopedic impairments.

Key Responsibilities

  • Conduct in-home physical therapy assessments and treatments
  • Develop individualized care plans focused on patient recovery and mobility improvement
  • Implement and adjust Home Exercise Programs (HEPs)
  • Provide patient and family education to promote optimal outcomes
  • Accurately document visits using EPIC and OASIS systems
  • Travel to patient homes within a manageable geographic area (max radius ~60 miles)

Qualifications

  • Licensed Physical Therapist in the state of Oregon (pending license accepted)
  • BLS (Basic Life Support) Certification required
  • 1+ year of physical therapy experience (Skilled Home Health experience strongly preferred)
  • COVID-19 primary vaccine series + one booster (required)
  • Valid driver's license and reliable transportation

Preferred Experience

  • Home Health setting
  • OASIS documentation system
  • Epic Charting (Remote Client and Rover App)

Orthopedic & Post-Op Cases:

  • Cervicalgia, Low Back Pain, Total Knee/Hip Replacement
  • Post-op Cervical/Shoulder/Spinal Surgeries

Neurologic Disorders:

  • Parkinson's Disease, MS, Muscular Dystrophy, Stroke Rehab, TBI, Cognitive Disorders

Professional Skills:

  • Home Exercise Programs (HEPs)
  • Patient & Family Education
  • Body Mechanics & Functional Mobility Training
