Job Description:
- Provide skilled physical therapy services to adult and geriatric patients in home settings
- Evaluate, diagnose, and develop personalized treatment plans to restore function and mobility
- Educate patients and caregivers on exercises and safe mobility strategies
- Document care using NetSmart EMR via a company-issued laptop at the point of service
- Collaborate with interdisciplinary teams to ensure comprehensive, quality care
- Use cell phone GPS for route navigation and communication with the team
Education & Certification Requirements:
- Master's Degree in Physical Therapy required
- Active Wisconsin PT license required
- BLS Certification (AHA) required
- Must have or be willing to obtain NPI number
- Valid driver's license and proof of auto insurance required
Minimum Experience:
- Home health experience preferred
- Strong background working with adult/geriatric patients with orthopedic, neurological, and mobility-related diagnoses
- Familiarity with physical therapy standards and best practices in home care environments
Charting & Equipment:
- Point-of-care documentation using NetSmart EMR
- Laptop and company-issued cell phone for charting and GPS navigation
