The program manager is responsible for communicating with referral hospitals, providers and clinical leaders in regards to patient transports and referrals to ensure quality of care. The manager will lead quality and process improvement program initiatives for the Access Center. In addition, this person will collaborate with a multidisciplinary team at Children's to facilitate the achievement of quality outcomes for patients/families. Provides outreach to outside hospitals in regard to follow up on identified opportunities and provides pediatric expertise.Responsibilities
- Develop and implement strategic initiatives to communicate with the referring facilities and physicians to increase the use of hospital facilities and services; develop relationships with referring physicians and local and outlying hospitals with the purpose of establishing and maintaining referral relationships, improving patient care and increasing referral patterns; identify needs within the physician's practices and referral facilities for medical inservices and facilitate information sharing between staff physicians and primary care physicians, support interdepartmental activities to increase Children's visibility in the community.
- Monitor Children's referral process to ensure communication is completed in an appropriate and timely manner.
- Document, analyze, and report referral facility and physician usage and trends and make recommendations regarding improvement and new service opportunities.
- Provide customer service to referral facilities and physicians who are experiencing difficulties with referring patients into the system through the Access Center in collaboration with Access Center leadership.
- Identify new opportunities to introduce Children's services and increase referral patterns; identify potential marketplace opportunities to develop educational partnerships with primary care physicians and healthcare workers.
- Complete case reviews for the quality and performance improvement programs for the Access Center and ensure all follow up is completed until loop closure on identified opportunities.
- In collaboration with the Quality, Safety and Outreach Manager identify opportunities for improvement and implement strategies until desired improvements are obtained within the Access Center.
- Provide follow-up to facilities that refer patients to Children's through the Access Center.
- Provides and maintains a Qaity Metric Plan for the Access Center and updates as needed and facilitates annual review.
- Lead quality and process improvement projects for the Access Center in collaboration with Access Center leadership.
- Maintains quality databases and ensures protection of confidential data.
- Responsible for the aggregation, monitoring and reporting of regulatory mandated and/or program specific quality metrics.
- Presents abstracts and posters at related conferences as the primary or co-presenter as well as mentors others in poster presentations.
Work Experience
- At least 3 years Transport, Access Center or Emergency ServicesRequired
- At least 5 years related experience in pediatricsRequired
EDUCATION
- Four-year Bachelor's degree or equivalent experience BSN
LICENSES AND CERTIFICATIONS
- Current RN license within State of Texas Req
- Quality certification as determined by VP within 2 yearsPreferred
