At Stony Brook Medicine, a TH CTS Senior Transplant Specialist / Transplant Coordinator is a valuable member of our team, who provides clinical services to our patient population. Qualified candidates will demonstrate superior patient care and possess outstanding communication skills while adhering to our high standard of excellence. Duties may include the following but are not limited to: Working with pre-transplant multi-disciplinary team to conduct new patient evaluations and re-evaluations of patients on annual basis. Coordinates patient entry and introduction to the transplant program. Coordinates pre-transplant education for patients and their caregivers. Works with the outreach coordinator to provide professional education for community nephrologists and dialysis center staff. Coordinates medical, surgical, financial, psychosocial, nutrition, and pharmaceutical evaluation and on-going care needs of pre transplant patients.
