This position reviews documentation from transition vendors, analyzes data to identify trends, and collaborates with Medicaid partners to ensure best practices. The Monitor also provides technical assistance, helps design training, and contributes to program and process improvements that strengthen community transitions. The role requires strong analytical, organizational, and communication skills, as well as experience with Home and Community-Based Services and the needs of the target populations. Position reports to the MFP Associate Director.
