Job SummaryThe Pre-Employment Transition Services (Pre-ETS) Case Manager provides direct support to students with disabilities, ages 14-22, to assist them in preparing for successful transitions from high school to post-secondary education, training, or competitive integrated employment. The Case Manager coordinates, monitors, and delivers Pre-ETS activities in partnership with schools, families, employers, and community service providers.

  • Serve as a primary point of contact for assigned students with disabilities receiving Pre-ETS services.
  • Conduct outreach to students, families, and schools to explain available Pre-ETS services.
  • Coordinate and facilitate the five required Pre-ETS services:
  • Job Exploration Counseling
  • Work-Based Learning Experiences
  • Counseling on Post-Secondary Education Opportunities
  • Workplace Readiness Training
  • Instruction in Self-Advocacy
  • Develop individualized service plans in collaboration with students, families, and educational staff.
  • Maintain regular communication with schools, families, and community partners to ensure seamless delivery of services.
  • Monitor student progress and document services provided in accordance with agency policies and funding requirements.
  • Assist students in identifying career interests, setting goals, and accessing community resources.
  • Coordinate or deliver group workshops, training sessions, and work-readiness activities.
  • Facilitate connections to internships, job shadowing, part-time employment, and other work-based learning experiences.
  • Ensure services are delivered in compliance with applicable laws, regulations, and program guidelines (e.g., WIOA, IDEA, ADA).
  • Maintain accurate, timely, and confidential case documentation and reports.
  • Attend staff meetings, training sessions, and professional development opportunities as required.

QualificationsEducation & Experience:
  • Bachelor's degree in Rehabilitation Counseling, Education, Social Work, Human Services, or a related field required.
  • Experience working with youth with disabilities, transition-age students, or vocational services preferred.
  • Knowledge of Pre-ETS services, transition planning, and community resources is highly desirable.
Knowledge, Skills, & Abilities
  • Strong understanding of the needs and barriers faced by students with disabilities.
  • Ability to engage and motivate youth in transition planning.
  • Excellent communication, collaboration, and problem-solving skills.
  • Strong organizational and time management skills.
  • Ability to work independently and manage a caseload effectively.
  • Proficiency in Microsoft Office and case management software.
  • Commitment to promoting inclusion and equitable opportunities for individuals with disabilities.
