Job Title: Transition Care Manager

Division/Program: Full Service Partnership - TAY - Sacramento

Starting Compensation: 71,760.00 - 86,112.00 USD Per Hour

Working Location: Sacramento: Downtown Center (In-Person Position)

Working Hours/Shift: Full-Time, Monday-Friday, AM Shift

What You Bring To SBHG (Qualifications):

  • Associate degree plus six years of experience in a mental health field, OR
  • Bachelor's degree in a Mental Health Field plus four years of experience in a mental health field, OR
  • Master's degree in a Mental Health Field plus two years of experience in a mental health field.
  • License or Certification
  • Level 42: BBS registration as Associate Marriage Family Therapist, Social Worker, or Professional Clinical Counselor or Psychology Assistant.
  • BBS licensed as LMFT, LCSW, LPCC, or Psychologist
  • Valid California Driver's License
  • Meets Agency driver eligibility criteria.

Note: Must possess a valid California Driver's License and maintain a driving record that meets the company driver's eligibility policy.

How You Will Make A Difference (Job Overview):

This position coordinates the care and leads the FSP team regarding service delivery based on participant voice.
