POSITION SUMMARY
The Aftercare Coordinator/Therapist plays a key role in supporting residents as they transition from treatment to long-term recovery. This position is responsible for assessing ongoing needs, enhancing motivation and commitment, creating personalized aftercare plans, and providing therapeutic guidance during the discharge process. The ideal candidate is clinically trained, resourceful, and committed to helping clients build sustainable recovery paths.
The primary responsibility is the coordination of meaningful, individualized aftercare development from admission through discharge in collaboration with the resident, their family, and referral sources. Aftercare will be developed in close coordination with the Clinical Team and as an integrated member of the Treatment Team.
In addition to aftercare coordination, the Aftercare Coordinator/Therapist is responsible for assisting in the daily operations of the Clinical, Admissions, and Business Development departments. This position will collaborate with the Director of Business Development to help manage and expand relationships with community, state, and national programs and resources utilized by Hammocks on the Edisto. The role includes facilitation of weekly aftercare groups, ongoing alumni follow-up, and alumni coordination in partnership with the Director of Business Development.
The ideal candidate understands the importance of first impressions, maintains a high standard of professional phone etiquette, and is committed to providing excellent service and communication to residents, families, and referral sources. This individual will also assist the Clinical team in offering additional services to residents and will receive supervision hours applicable toward clinical certification or licensure.
KEY RESPONSIBILITIES
Aftercare Planning
- Collaborate with clients and the multidisciplinary treatment team to develop individualized aftercare plans.
- Coordinate referrals to outpatient programs, therapists, sober living homes, vocational support, and other recovery resources.
- Ensure continuity of care by securing follow-up appointments and support services prior to discharge.
Therapeutic Support
- Provide individual and group therapy sessions focused on preparing for life after treatment.
- Facilitate educational workshops and family meetings addressing relapse prevention, recovery maintenance, and reintegration.
- Support clients in identifying goals and coping strategies for post-treatment success.
Collaboration & Communication
- Function as a liaison between the treatment center, families, community providers, and other stakeholders.
- Maintain consistent communication with outpatient clinicians and support networks to support seamless transitions.
- Participate in treatment team meetings and contribute to care planning.
Documentation
- Complete clinical documentation in a timely and accurate manner within the electronic medical record (EMR).
- Maintain records that comply with legal, ethical, and accreditation standards related to aftercare planning and therapeutic services.
Resource Management
- Maintain and update a directory of community resources including housing, employment, outpatient care, peer support, and mental health providers.
- Monitor client engagement with post-discharge services and provide follow-up as needed.
QUALIFICATIONS
Education & Experience
- Master's degree in counseling, Social Work, Psychology, or a related behavioral health field preferred.
- Bachelor's degree in a human services-related field with a current certification in addiction counseling (e.g., CADC, CAC, NCAC) will be considered.
- Minimum of 1–2 years of experience in addiction treatment, aftercare planning, or clinical case management.
- Familiarity with local and national recovery resources and treatment continuums.
Licensure/Certifications
- Licensure and/or certification required (e.g., LCSW, LISW, LMFT, LPC, LPC-A, LAC, ADC, AADC) per state regulations.
Skills
- Strong clinical judgment and counseling skills.
- Excellent multitasking and organizational abilities, especially in managing complex cases and time-sensitive care transitions.
- Ability to organize complex care plans and manage multiple cases effectively.
- Compassionate and culturally sensitive communication style.
- Competency with EMR systems and confidentiality practices (e.g., HIPAA).
Working Conditions
- Flexible schedule based on client needs and program demands.
- May include a 4-day work week option.
- Occasional evening or weekend availability may be necessary.
- Requires interaction with individuals in various emotional states and recovery phases.

- Accept responsibility for professional growth and development of self and Hammocks on the Edisto.

- Attend all mandatory facility in-services and staff development activities, as scheduled.
- Adhere to facility standards concerning conduct, dress, attendance, and punctuality.
- Support facility wide performance improvement goals and objectives.
- Maintain confidentiality of facility employees, individuals who make inquiries to the facility and former and current patients.
WORKING CONDITIONS/PHYSICAL DEMANDS
- Local travel for community outreach, public relations, and marketing activities (when necessary).
- Direct and indirect patient contact, as deemed appropriate.
- Work in an office environment and be able to tolerate gradient levels of standing, walking, stooping, and lifting to 10 pounds.
- Tolerate stressful situations and various conditions unpleasant to the sense of sight, touch, and smell.
- Ability to adjust to large fluctuations in workload and willingness and able to assist with other duties during times of decrease in workload.
