GENERAL SUMMARY Provides direct services to individuals through the implementation of service and support plans, documentation of data and other means assigned and, as established by the planning team to address habilitative, therapeutic and behavioral needs. This includes the requirement to maintain medication certification, and administer medications, as assigned. Team Leaders may be assigned to work with any individual served by the Day Habilitation Program. They are required to administer medications as part of any day habilitation team, on an ongoing or short-term basis. COMPANY STANDARDS
- Maintains regular attendance and punctuality in order to provide quality direct support and service to individuals served. This position performs all hours onsite and at all assigned physical locations of service.
- Follows all safety rules and regulations; wears and/or uses required protective equipment while working; immediately reports any safety concerns or unsafe working conditions to Day Habilitation management.
- Conducts all aspects of direct support to individuals served in compliance with the JM Murray mission statement, policies and procedures, professional standards, and applicable governmental laws, rules and regulations.
ESSENTIAL FUNCTIONS
Prepares, communicates and follows daily schedules and assignments for assigned team and ensures that tasks are completed. Ensures adequate and appropriate materials are available and organized so that effective services are delivered. Performs necessary direct support duties to ensure individual safety, health, hygiene and development. Duties include support with dining, personal care and hygiene needs, assistance to enter and exit buses/company vehicles, etc., and tasks and activities specific to individual goals as outlined in the Staff Action Plan. Follows and maintains daily work assignments, as scheduled at the start of each day, providing coverage for direct services in various areas including at community locations, as assigned. Certified to administer medications for any day habilitation team or individual, as a regular or short-term assignment. Works cooperatively with coworkers to ensure individuals receive appropriate services such as adherence to specified diets, appropriate response to injuries, seizures or illness, etc. Provides safe transportation as assigned; ensures Company vehicles are maintained including keeping them clean inside and free of debris, have gas in them, and first aid kits are on hand and stocked; immediately reports any mechanical, safety or other needs for repair and maintenance. Completes appropriate and timely documentation of services which includes the use of the JM Murray Electronic Medical Record, and the use of equipment such as a computer, laptop, and/or a tablet. Provides Supervisors with information and feedback regarding individual staff action plans and safeguards. Assists with developing and implementing clubs, special events, and placements for individuals served. Performs clerical duties necessary to maintain accurate records and service operations including but not limited to filing, copying, computer entry, organizing and ordering supplies, cleaning the team rooms and common areas, laundry, etc. Attends all required and requested trainings and meetings, and maintains certifications for required training including but not limited to Medication Administration, CPR, First Aid, AED and SCIP. Conducts self in a manner as to always contribute to maximum individual growth and in accordance with all regulations, the DSP Code of Ethics and Company policy and procedures. Represents JM Murray and the JM Murray mission in a positive manner, treating people with respect and dignity. Performs additional duties and responsibilities, as assigned. Ability to work both independently or as part of a team Sound decision-making skills and judgment Good verbal and written communication skills and efficient use of technology Compassionate and caring OTHER REQUIREMENTS Requires a valid NYS Driver's License that meets Company standards; requires proof of NYS minimum insurance coverage. Requires clearance through a Background Check process that will include, but is not limited to, clearance through the NYS Justice Center, the Office of People with Development Disabilities (OPWDD), the NYS Office of Child and Family Services, and the NYS Department of Motor Vehicles. WORKING CONDITIONS Job assignments may take place indoors and outdoors and require travel to various community locations, in various environmental conditions including rain, snow and cold weather.
