Supportive housing—affordable housing paired with services designed to help people maintain their homes long-term—is recognized as a proven and cost-effective solution to chronic homelessness.

But we don't stop at providing housing. Breaking Ground's programs and services assist people experiencing street homelessness—especially those who have been on the streets the longest—to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get and stay on the path to a permanent home.

Breaking Ground's Street to Home program makes contact with over 2,000 street homeless individuals annually and connects more than 500 individuals to housing, medical and mental health services, substance abuse counseling, and other supports each year. The personalized attention clients receive ensures that more than 95% of those placed remain stably housed.

Reporting to the Assistant Director, Programs, Outreach, the Team Leader II, Outreach, works with street homeless individuals. The Team Leader II supervises a team of Housing Advocates who employ Harm Reduction and Housing First approaches during street outreach, engaging individuals to assess their homelessness history, needs, and placement options.

The Team Leader II is responsible for understanding the homeless population in their catchment areas and collaborating with community partners to implement suitable housing and service solutions. They manage their team's response to community concerns, oversee Code Red/Blue alerts, identify new hotspots and encampments, and handle all 9.58 assessments and removals during their shift.

This position involves a 35-hour workweek.

ESSENTIAL DUTIES AND RESPONSIBILITIES :
  • Supervise Housing Advocates, including scheduling, weekly supervision, providing guidance, clinical direction, and ensuring effective street outreach.
  • Complete 9.58 certification and oversee all assessments and removals for the shift.
  • Serve as a liaison with community partners, following up on homeless conditions, community concerns, 311 calls, and encampment engagement.
  • Develop strong collaborative relationships with external partners, service providers, NYPD, EMS, and other stakeholders.
  • Oversee and participate in Code Blue/Red alerts.
  • Ensure accurate documentation and timely reporting.
  • Attend onsite meetings with community partners.
  • Perform other duties as assigned.
MINIMUM QUALIFICATIONS:
  • NYS Licensed Masters Social Worker (LMSW) required.
  • Previous clinical, supervisory, and administrative experience preferred.
  • Understanding of Housing First, client-centered motivational interviewing, Harm Reduction models, behavioral and medical health systems.
  • Experience with crisis intervention, risk assessments, and incident management.
  • Excellent verbal and written communication skills; ability to work with diverse populations.
  • Strong collaborative skills; ability to work successfully with a wide range of stakeholders.
  • Detail-oriented with the ability to manage team tasks.
  • Valid driver's license from NY, NJ, CT, or PA with a good driving record, as defined by Breaking Ground, is required.
  • Proficiency with Microsoft Office Suite (Word, Excel, Outlook, Teams).
