In this role, you will empower individuals with developmental disabilities by collaborating with their support networks to identify, access, and utilize both paid services and natural supports. Your goal is to enhance their independence, productivity, and integration in alignment with their unique lifestyle preferences and needs.

Your responsibilities will include:

  • Creating, updating, and reviewing the Person-Centered Support Plan in collaboration with the individual and their support network.
  • Monitoring the quality of services provided to those we serve.
  • Documenting individual progress and maintaining accurate records of services.
  • Recording daily billable activities.
  • Providing support and guidance regarding available community services and supports.
  • Connecting individuals with natural supports and community resources, while facilitating access to necessary services and entitlements.
  • Adjusting service systems as needed to improve accessibility for individuals.
  • Conducting ongoing monitoring and follow-up to ensure effective implementation of person-centered plans.
  • Assisting with transition planning and service arrangements for individuals moving to new locations.
  • Adhering to the Rules of Conduct for Case Managers serving individuals with developmental disabilities.

Schedule: Typically Monday through Friday, 9 AM to 5 PM, with occasional flexibility to accommodate family needs. This is a hybrid role that combines working from home and engaging with the community.

Requirements:

  • A minimum of six months of experience in human services, along with a Bachelor's degree or equivalent experience in developmental disability services.
  • A minimum of six months of full-time experience in human services, with a Bachelor's degree or equivalent experience in developmental disability services (substitutable, at a rate of six months of full-time experience for each missing semester of college).
