TARGETED CASE MANAGER - KidsPeace Children's Hospital : Job Details

The Targeted Case Manager s purpose is to identify the medical, social, educational and other needs of the eligible family, identify the services necessary to meet those needs, and facilitate and coordinate access to those services. The Targeted Case Manager is responsible for the overall management of assigned caseloads of children/families served by KidsPeace New England. QUALIFICATIONS: Level I Case Managers: Meets the qualifications for Level II Case Manager OR be any agency staff who has parented a child or adolescent with special needs and is able to meet specified staff, training and supervision requirements. Approval from BDS Children s Services must be maintained in the personnel file. Level II Case Managers: a.
