Florida Department of Health Office of Information Technology Formal Scope of Work Systems Administrator SOW24-124 1. PURPOSE: This scope of work is for Systems Administrator consulting services to support the development, implementation and life-cycle management of new and existing enterprise technologies required to support the Florida Department of Health (Department), Office of Information Technology, Bureau of Enterprise Foundational Services, Cloud Support Team. Contractor will provide these services to the Department. 2. TERM: This scope of work will begin on 7/1/2024 or the date on which the purchase order is issued, whichever is later. It will end at midnight, Eastern Time on 6/30/2027. The State of Florida's performance and obligation to pay under this purchase order and any subsequent renewal is contingent upon annual appropriation by the Legislature and satisfactory performance of the Contractor. 3. LOCATION OF WORK: The worksite for this scope of work is the following location(s): Florida Department of Health Office of Information Technology 4. CRIMINAL BACKGROUND SCREENING: 4052 Bald Cypress Way Tallahassee, FL 32399 The Department will conduct a criminal history record check, including fingerprinting, on the consultant ssigned by the Contractor prior to the consultant commencing work for the Department. The Department retains sole discretion as to whether the results of the criminal history record check will result in the Contractor's employee being disqualified from performing services pursuant to this agreement. 5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE: Contractor staff assigned to this agreement must possess the following minimum qualifications and experience: 5.1. 5 years' experience with enterprise class Windows Server system design and administration, including the proactive monitoring of system environments and problem resolution, user support, system pplication support, operating system (OS) support, hardware/software system installation, configuration, and maintenance. 5.2. 5 years' experience as an enterprise administrator in Active Directory and Azure Active Directory domain infrastructure. 5.3. 5 years' experience administering Microsoft Exchange or Exchange online. 5.4. 5 years' experience administering VMWare and management of systems on the VMWare platform. 5.5. 2 years' experience designing, implementing, and administering Microsoft Azure or Amazon AWS cloud solutions. 1 1/29/2024 Florida Department of Health Office of Information Technology Formal Scope of Work Systems Administrator SOW24-124 5.6. 5 years' experience managing and supporting systems using various scripting languages including Microsoft Powershell. 6. CONTRACTOR RESPONSIBILITIES: 6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified: 6.1.1. Design, install, maintain, and document all server and cloud platform requirements in an enterprise scale Microsoft Azure Active Directory cloud platform as directed by the Department. These duties include but are not limited to: system administration, server maintenance, monthly patching, daily monitoring, error tracking, issue remediation and change control. 6.1.2. Perform proactive daily monitoring for high availability, system data protection, central uthentication, user account management, disk storage management, hardware and OS maintenance, and application maintenance for all platforms serviced by the Enterprise Cloud Support Team, as directed by the Department. 6.1.3. Investigate, troubleshoot, and resolve issues reported by customers, system administrators, and support teams daily as directed by the Department. 6.1.4. Provide technical expertise to users, management, vendors, and peers for issues involving other Department business offices and vendors daily as directed by the Department. 6.1.5. Provide documented resolutions to Service Desk and Change Management tickets, daily, as directed by the Department. 6.1.6. DATA SECURITY AND CONFIDENTIALITY TASK: The Contractor, its employees, subcontractors, and agents must comply at all times with all Department data security procedures and policies in the performance of this scope of work as specified in the Data Security and Confidentiality document attached to the purchase order. 6.2. DELIVERABLES: Contractor will complete and submit the following deliverables to the Department in the time and manner specified: 6.2.1. Monthly: Provision of System Administrator services in the time and manner specified in Tasks 6.1.1 through 6.1.5. 7. METHOD OF PAYMENT: 7.1. A purchase order will be issued to the Contractor. 7.2. The method of payment for this purchase order is unit rate. 7.3. The Contractor will not receive payment in advance for goods or services described in this scope of work. 7.4. The Contractor must submit an invoice monthly that provides a detailed accounting of the deliverables performed during the invoice period for which payment is being requested. 7.5. PERFORMANCE MEASURES AND FINANCIAL CONSEQUENCES: ll deliverables and related tasks must be completed 100% as specified. Failure to satisfactorily complete or submit a deliverable in the time and manner specified will result in a financial consequence as indicated below: 8.1. Failure to complete and submit Deliverable 6.2.1. in the time and manner specified will result in a payment reduction equal to 20 percent of the total monthly invoiced amount. 8.2. Failure to comply with Tasks 6.1.6, DATA SECURITY AND CONFIDENTIALITY will result in a payment reduction equal to 20 percent of the total monthly invoiced amount for each week the Contractor is not in compliance. 9. CONTRACTOR TRAVEL REIMBURSEMENT: Reimbursement for travel expenses is authorized only when approved in advance by the Department Contract Manager and conducted in accordance with Section 112.061, Florida Statutes and Department Internal Operating Procedure (IOP) 56-37. 10.
