Reports to the VP of Nursing & Operations. Responsibilities include cultivation of relationships with case management, discharge planners, clinical social workers, and other key referral sources by providing a single point of contact to ensure seamless services throughout the referral process. Meets with patients/family/significant other to assess need for Swing Bed Program. Capable of explaining the Swing Bed Program including medical criteria for admission and knowledge of reimbursement process. Actively maintains timely market intelligence to ensure the organization can serve the community with the appropriate type of programs. Acts in a troubleshooting capacity regarding physicians' issues or potential issues. MISSION, VALUES and SERVICE GOALS

Essential Functions and Responsibilities:
  • Collects patient information during the referral process.
  • Educates patient/family/significant other on Swing Bed Program
  • Accepts patients on behalf of the facility after clinical and financial review.
  • Communicates with and educates referral sources regarding Swing Bed Program
  • Works with hospital discharge planners to facilitate a smooth discharge process.
  • Facilitates nurse to nurse communication between the facility nursing staff and hospital/assisted living nursing staff to assure continuity of care.
  • Provides a monthly report on the number of cases sent to the facility, discuss the process, and modify any necessary areas.
  • Serves as a liaison between referral sources, patients, and families.
  • Prepares reports for the leadership team as requested.
  • Adheres to work schedules in completing and performing assigned tasks.
  • Demonstrates regular communication with referral sources to establish working relationships.

  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies, and department specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR), and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.

Education and Experience
  • The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of an associate degree or Professional Training. RN, BSN BSW/MSW preferred. Prefer associate degree in health-related field. A minimum of 5 years of job-related experience either as an RN or case manager. Clinical experience appropriate to designated patient population; management or similar experience in relating across disciplines in complex healthcare environment but willingness to think outside the box. After January 1, 2014, candidates are required to have or obtain a BSN within five (5) years of employment as a Registered Nurse or will have to option to obtain certification in area of specialty. The certification must be maintained based off accrediting body standards.

  • Ability to communicate positively and effectively with all levels of participants in health care delivery in both formal and informal settings and with individuals as well as groups of varying size.
  • Demonstrates well-developed knowledge of Beacon Health System's departmental operations and how they interact with each other.
  • Requires in-depth knowledge and understanding of physician issues, concerns and referral patterns.
  • Requires the analytical, problem solving, project implementation or management and organization skills necessary to identify problems and deliver solutions in a timely manner and design training programs. Also requires the ability to work independently.
  • Requires proficient computer skills and the ability to utilize various software programs (Microsoft Office: Word, Excel, databases, etc.). Also requires the ability to develop and prepare reports.
  • Demonstrates the leadership skills necessary to elicit cooperation and support from others and a strong customer service focus.
  • Demonstrates the interpersonal and communication skills (both verbal and written) necessary to communicate effectively with staff and physicians and establish and maintain effective working relationships. Also requires the ability to pro-actively approach others regarding their level of understanding and educational needs. Good listening skills are essential.
  • Demonstrates the ability to assess training needs. Also requires the ability, (i.e., negotiating, persuading, counseling, etc.) to provide effective training in a wide variety of situations. Also demonstrates knowledge of adult education techniques and an interest in teaching others.
  • Demonstrates personal initiative, poise, confidence, persuasive negotiation skills and the willingness to accept challenges.

Working Conditions
  • Works in a patient care and office environment requiring the ability to prioritize workload in order to meet deadlines.
  • Travel to physician offices is required.
