About the Supports Coordinator - Case Manager Role As a Supports Coordinator within our Intellectual and Developmental Disabilities (IDD) service line, you'll play a vital role in supporting individuals with IDD, including those with Autism, in living self-directed, fulfilling lives. You'll build long-term relationships through ongoing case management, advocacy, and coordination of meaningful services. This position is ideal for someone who is compassionate, detail-oriented, and thrives in community-based environments where collaboration and relationships matter. Starting Rate: $19.89/hour, per current union agreement Supports Coordinator - Case Manager Core Responsibilities
  • Assess individual strengths, needs, and life goals
  • Develop person-centered plans in collaboration with the individual and their circle of support
  • Coordinate services that align with individuals' goals and ensure timely access to supports
  • Monitor service delivery and adjust supports to remain responsive and effective
  • Document all activities and communications according to regulatory and agency standards
  • Support families with tools and guidance to promote informed decision-making
  • Engage in ongoing professional development and training
What You Bring
  • Bachelor's degree with at least 12 credits in social sciences (e.g., social work, psychology, sociology, criminal justice); OR
  • A minimum of 12 credits in social sciences plus 2 years of related experience in human services
  • Ability to work independently and manage a dynamic caseload
  • Strong interpersonal, organizational, and communication skills
  • A valid driver's license and reliable transportation
