SUBSTANCE ABUSE COUNSELOR - Meadville Medical Center : Job Details

Full Time. Days. 9:00 a.m. - 5:00 p.m. Weekend rotation required. Maintains communication with other members of the chemical dependency treatment team. Acts as Case Manager for patients, functioning as a social worker when necessary to prepare patients for discharge. Counsels patients on an individual basis. Conducts group therapy as primary facilitator. Prepares Master Treatment Plans, Psychosocial interview forms and meaningful Aftercare Plans in concert with the needs of the patient. MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED A minimum Bachelor's Degree in counseling, psychology, education, social work or other behavioral science curriculum, Master's Degree preferred, or have earned a Pennsylvania Certified Alcohol and Drug Counselor (C.
