You will assist our holistic care team to guide our patients and their families through the intricacies of advanced illness, the end-of-life journey and offer bereavement assistance after the death of a loved one.This position requires flexibility as you care for patients virtually, at home, or in-patient settings. Further, at Guaranteed, our Spiritual Care Counselors emphatically honors each patient's spiritual beliefs and respect the unique cultures and traditions that make up our diverse community.Job ResponsibilitiesConduct a spiritual assessment within 5 days of admission to the hospice programAssist in development and implementation of the interdisciplinary patient care plan as it pertains to spiritual careProvide spiritual support in accordance with on-going wishes and needs of the patient and familyMaintain and submit written clinical records including the initial evaluation, the care plan and daily notesEvaluate the patients and family's response to, and effectiveness of, the spiritual counselingConfirm, on a weekly basis, the scheduling of visits with the patients' Case Managers to facilitate coordination of other staff visitsParticipate in IDG and QAPI meetings and activitiesProvide spiritual and bereavement support to staff as neededProvide consultation and education to the IDG on spiritual careProvide funeral or memorial services for patient's as requestedSkills and Toolset You'll Need to BringOrdained, commissioned or credentialed by an organized religious groupComfortable in an interfaith settingAbility to accept different lifestyles, cultures, beliefs and valuesMust be a licensed driver with an insured automobile in good working orderThe ability to communicate well, both verbally and in writingTechnical savviness: utilization of tablet, multi-line telephone, copy machine and other office equipmentAssume responsibility for self-development by continually striving to improve your Spiritual practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and readingRelevant ExperienceBachelors or higher degree from an accredited seminary or school of theologyBachelors of Divinity, Masters of Divinity or equivalent theology degree or trainingMinimum of one (1) year of experience in a health care setting or with death and dyingHospice experience is a plusExperience providing funeral or memorial services for patients as requestedConduct or participate in the memorial services for employees, facilities, and family members as required and requestedGuaranteed is a proud Equal Opportunity Employer.
