POSITION SUMMARY The Hospice Spiritual Care Coordinator provides spiritual support and counseling to patients and families facing terminal illness. The Spiritual Care Coordinator works as part of the interdisciplinary hospice team to address the emotional, spiritual, and existential needs of individuals of all faiths or no faith, respecting diverse beliefs and values. PRIMARY RESPONSIBILITIES

  • Spiritual Counseling:
    • Offer emotional and spiritual support to patients and their families.
    • Facilitate conversations about meaning, legacy, forgiveness, and grief.
    • Conduct spiritual assessments and develop care plans.
  • Interfaith Support:
    • Provide non-denominational support or align patients with spiritual leaders of their specific faith traditions if desired.
  • Ceremonial Services:
    • Conduct or coordinate religious services, funerals, memorials, and bedside rites or blessings.
  • Team Collaboration:
    • Participate in interdisciplinary team meetings.
    • Communicate patients' spiritual needs and coordinate with other hospice professionals.
  • Documentation:
    • Maintain accurate records of visits, assessments, and interventions in compliance with hospice policies and regulations.
  • Community Outreach:
    • Build and maintain relationships with local faith communities and clergy.
QUALIFICATIONS
  • Master of Divinity or equivalent theological degree preferred.
  • Ordination or endorsement by a recognized faith group.
  • Clinical Pastoral Education (CPE) units (often at least 1-2 units; 4 units preferred).
  • Certification or eligibility for certification as a Chaplain (e.g., APC, NACC).
  • Experience in hospice, palliative care, hospital, or pastoral care setting.
  • Strong interpersonal, listening, and communication skills.
  • Ability to work with people of diverse backgrounds and beliefs.
Work Environment & Physical Demands
  • Work is conducted at both a typical office environment and on-site at assigned facilities, engaging with residents, families, and staff.
