Speech Language Pathologist - Dignity Health : Job Details

The Therapist provides patient care including evaluating the patient for treatment providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management while delivering quality patient services.#AZEVREHAB - Qualifications. Minimum: Internship or rotation in appropriate clinical setting required. Graduate of an accredited Speech/ Language Pathology program or related field required. Computer skills required. Arizona License in professional area. American Heart Association BLS required. CT - Preferred: 2 years progressive experience in clinical area performing patient clinical care preferred. Masters or Doctorate degree in appropriate clinical area preferred. Prior use of electronic health record preferred.
