Job Title: Speech-Language Pathologist (SLP)

Shift: 9:00 AM – 5:00 PM
Contract Duration: 13 Weeks
Position Summary
We are seeking a licensed Speech-Language Pathologist to provide comprehensive assessment, diagnosis, and treatment for speech, language, cognitive-communication, and swallowing (dysphagia) disorders in a geriatric, medically complex population within a skilled nursing facility (SNF) and subacute care setting. This role emphasizes interdisciplinary collaboration, regulatory compliance, and quality patient outcomes.
Key Responsibilities
- Conduct thorough evaluations within 24 hours of physician referral, assessing speech, language, cognitive-communication, and swallowing abilities.
- Develop individualized treatment plans with measurable goals, evidence-based interventions, and regular progress adjustments.
- Deliver direct therapy, including compensatory strategies, cognitive-communication interventions, swallowing therapy, and group therapy as appropriate.
- Accurately complete documentation, including:
- Evaluation reports
- Daily treatment notes
- Weekly progress updates
- 14-day physician progress reports
- Discharge summaries
- Billing documentation and Plan of Care (POC) logs
- Educate residents, families, and nursing/CNA staff on communication techniques, safe swallowing strategies, and home-exercise programs.
- Coordinate with the interdisciplinary team (nursing, physicians, PT/OT, dietary, social work) to optimize outcomes and support discharge planning.
- Participate in quality improvement initiatives, ensuring adherence to Medicare/Medicaid documentation standards (PDPM, Title 22, CMS).
- If supervising SLPAs or therapy aides:
- Meet California supervision requirements (≥2 years experience or 3,120 hours).
- Provide direct supervision for 20% of duties during the first 90 days.
- Maintain CEUs in supervision.
Qualifications
- Master's degree in Speech-Language Pathology (accredited institution).
- Current California SLP license (RPE accepted if in process).
- Certificate of Clinical Competence (CCC-SLP) from ASHA preferred.
- Experience in SNF, subacute, or geriatric rehabilitation settings preferred.
- Knowledge of Medicare/Medicaid billing, PDPM regulations, Title 22, and CMS guidelines.
- Strong clinical assessment, treatment planning, documentation, and communication skills.
- Current BLS/CPR certification.
Work Environment & Expectations
- Serve a diverse geriatric population including patients with stroke, dementia, cognitive-communication deficits, and swallowing disorders.
- Typical caseload includes both subacute rehab (Medicare Part A, up to ~100 days) and long-term care residents.
- Productivity expectations: 80–90% of scheduled hours billed to therapy.
- Workload may fluctuate: periods of lower intensity followed by days with multiple new admissions and high treatment demand.
- Effective collaboration with nursing/CNA staff is essential. Flexibility and adaptability are key.
