Spanish-Speaking Nurse Practitioner (In-Office Telehealth - Home Care Patients) Location: Boro Park, Brooklyn, NY Schedule: Monday to Friday, 9 AM - 5 PM Salary: $80-$85 per hour Start Date: ASAP We are looking for a Spanish-speaking Nurse Practitioner to join our team in Boro Park, conducting telehealth visits for home care patients from within our medical office. This is not a remote role and all work is performed on-site in a structured, team-oriented environment. About the Role: You will be working closely with our care coordination and home health teams, managing a panel of homebound patients through scheduled telehealth visits. These patients are primarily elderly or chronically ill individuals receiving care in their homes through licensed home care agencies. Types of visits include:

• Initial telehealth evaluations for home care services
• Follow-up visits for chronic condition management (e.g., hypertension, diabetes, COPD)
• Medication management and reconciliation
• Reviewing and updating care plans
• Coordinating with home care nurses, aides, and social workers
• Addressing acute issues and determining next steps (e.g., home visit, in-person follow-up, ER referral if needed)

Requirements:

• Licensed Nurse Practitioner in New York
• Fluent in Spanish (required)
• Must have active Medicaid billing credentials
• Experience with telehealth and managing chronic conditions is preferred
• Familiarity with CyberMed EMR is a plus
