The Acute Care Case Manager evaluates the patients health status and the patient/family/significant others care capabilities to facilitate the discharge planning process. The case manager develops a multidisciplinary, culturally appropriate, age-specific discharge plan. They also complete clinical reviews utilizing Internal criteria for the appropriate level of care and insurance coverage. What Youll Do:Assesses and identifies patients needs during the discharge planning process. Provide resources and community referrals for continuity of care. Care coordination with families, providers, and clinical team. Utilization Reviews. Who You Are: Bachelor's degree in a healthcare-related field or equivalent experience. ASN/ BSN preferred but not required 3 5 years of case management-related health care experience. A minimum of 7 years of experience is required without a bachelors degree.
